In May 2005 Ufi Limited’s internal audit procedures identified a potential issue that could be traced to the fact that the main online learndirect system (Learner Support Environment, or “LSE”) did not enforce unique learner passwords. Although the LSE required a learner to enter a user name followed by a password each time they accessed the system, the LSE did not prevent other users from also using the same password. As is common in many information systems the user name was directly related to the learners name which meant that if the password became known it was possible to enter a learners learning account.
